You must complete another 'Petition for Change of Name" form

You must complete another 'Petition for Change of Name" form as shown in the example PDF below. Please be sure to read and examine the example below carefully.

Case Status: Closed

After the process is complete, the case status should reflect "Closed" as shown in the picture, and not "Inactive" which means the case is still open.
